Cancelling An Appointment

If you are unable to keep your appointment, or if you will be delayed, please notify the office as soon as possible.  This courtesy allows us to be of service to our other patients.

Referrals

If another doctor has asked you to see us, please let us know so that we may report to him or her promptly.

Confidentiality

Your medical record is strictly private.  No information regarding your condition will be given to your employer, friends or relatives without your permission.  The only exception to this is when required by law, as in industrial injuries, assault, etc.

Doctor-Patient Relations

These are best when based on mutual understanding.  We will make an effort to explain everything to you about your condition.  If you ever have complaints about your care or fees, please discuss them with the doctor or with the office manager.


New Patients

Prior to your visit:

Please bring the following items with you: a current insurance card, photo identification, method for payment for any co-pay or deductible required by your insurance company and any CT/MRI scans or other lab tests that pertain to your visit.  You should receive an information packet in the mail prior to your appointment.  Your visit will go more quickly if you fill out all paperwork in the reception packet prior to the day of your visit.  Those forms are located here if you would like to download them.  

On the day of your visit:

Please check in at our front desk with the receptionist.  You will be taken to the examination room where the nurse will prepare you for the doctor’s visit.  Please let us know what pharmacy you would like to use in the event we prescribe any medication for you.  We would like to know if you were referred by a physician so we can contact them in a timely fashion.  We will arrange for any surgical scheduling, appointments with other physicians, labs or tests prior to you leaving whenever possible.

After your visit:

Your doctor will dictate a note regarding your visit and send a copy of this to the referring physician and/or primary care physician.  If you have any tests or labs ordered, we will contact you after receiving these to let you know the results or to schedule a follow-up appointment to go over these with the physician.  If you have not heard anything within a week after your test, please contact us to see if there has been a problem obtaining these results.  Please contact us if you will not be able to keep your follow-up appointment as we can reschedule your visit and be of service to another patient during that time.
