General Office Policies

• First time patients are asked to come to the office at least 15 minutes prior to their appointment to complete any necessary forms.  Your evaluation and treatment will be facilitated by completing the patient information forms.  Please let the doctor know of any medications being taken, any allergies or other medical problems.  Please bring your insurance forms, a current insurance card and photo ID. 

• If you are unable to keep your appointment, or if you will be delayed, please notify the office.  This courtesy allows us to be of service to our other patients.

• If another doctor has asked you to see us, please let us know so we may report to him promptly.

• Your medical record is strictly private.  No information regarding your condition will be given to your employer, friends or relatives without your permission.  The only exception to this is when required by law, as in industrial injuries, assault, etc.

• These are best when based on a mutual understanding.  We will make an effort to explain everything to you about your condition.  If you ever have complaints about your care or fees, please discuss them with the doctor or with the office manager.  Thank you.
